This policy explains how Hope Connection Therapy Company, PLLC handles information connected with this public website.

Please do not use this website for emergencies or detailed clinical information. The public request form is for administrative consultation requests only.

Information collected through the request form

The consultation form currently collects your name, email address, phone number, who is seeking services, Florida-location confirmation, preferred therapy format, payment method, therapist preference, general availability, preferred follow-up method, and consent to be contacted.

Please do not submit diagnoses, treatment history, insurance identification numbers, detailed symptoms, safety concerns, or other protected health information through this public form.

How Formspree inquiries are delivered

The form is processed by Formspree and delivered to Charity Cannavo’s Hope Connection email account. Charity may forward an inquiry to Angela Cash when appropriate for follow-up. The public form is not a secure clinical-message channel and is not intended for protected health information.

Hope Connection generally responds within 1–2 business days. Hope Connection deletes its email copy after the consultation process is complete. Formspree may retain submission data according to the practice’s Formspree account settings and Formspree’s own privacy policy.

Clerie, Spruce, and established-client information

The website form does not automatically create a Clerie client record. If you move forward with services, Hope Connection may separately create the necessary administrative record in Clerie. Hope Connection uses Clerie for established-client scheduling, paperwork, and client records, and may use Spruce for practice communication.

How inquiry information is used

Information submitted through the website may be used to respond to your request, arrange a free consultation, determine basic location or payment fit, review general availability, and identify an appropriate next step. Hope Connection does not sell website inquiry information.

Correction and deletion requests

You may ask Hope Connection to correct or delete its copy of a website inquiry by emailing charity@hopeconnection.net. A request may be limited when retention is required by law or when the information has already been moved into a clinical or administrative record governed by a different retention requirement.

Analytics, fonts, and technical information

This website does not currently use Google Analytics, advertising pixels, or behavioral advertising tools. The website host may process standard technical logs under its own settings and policies. The site loads fonts from Google Fonts, so Google may receive technical request information such as an IP address and browser details when those fonts load.

Cookies and external links

Hope Connection does not intentionally place advertising cookies. Third-party services and websites may use their own cookies or similar technologies and are governed by their own privacy policies.

Children and teen services

The website is not directed to children under age 13. Hope Connection provides services to teens age 13 and older. A parent or legal guardian should submit an initial website inquiry for a minor when appropriate.

Clinical privacy notices

This website policy describes public website practices. It does not replace any Notice of Privacy Practices, informed-consent document, or explanation of therapy confidentiality provided during intake.
